MAIN PURPOSE OF JOB: Ensuring provision of quality integrated Non-communicable Disease care (prevention, screening and Management services) both at facility and community level; participate in other program activities such as M&E, report and proposal writing, Continuous quality improvement (CQI), planning and coordinating training/mentorship and operations research. PRINCIPAL RESPONSIBILITIES Co-ordinate hypertension Care Provision Ensuring provision of quality integrated hypertension (and NCD) services including awareness, screening and treatment at the Healthy Heart Africa project supported sites both at facility and community level. Ensuring that clients with hypertension (and other NCDs) are promptly linked to care, initiated on treatment, retained and actively followed up to achieve BP control. Overseeing adherence counselling activities for clients with hypertension (and other NCD). Ensure hypertension (and other NCD) services provided are in line with the National guidelines. Carry out regular supportive supervision to all the assigned facilities and prepare progress reports. Patient management Support clinicians to provide quality patient management for clients with hypertension and other health conditions as required Provide regular on-job mentorship to health care workers at the supported sites Capacity building, supportive supervision and mentorship Organize and conduct trainings for the health care workers at Amref Health Africa in Kenya supported sites both at the community and facility level on hypertension care and other related areas as required in line with the Ministry of Health guidelines. Carrying out on-job training, Continuous Medical Education (CMEs) and mentorship for health workers. Planning joint supportive supervision sessions together with the Sub-county health teams and CHEWs to the project supported sites. Ensure the availability of Ministry of Health hypertension (and NCD) management protocols and SOPs at the supported health Supervise Facility based Community Health Volunteers activities including screening, linkage, documentation and data entry as per their SOPs. Ensure the CHVs and other health facility staff assigned upload necessary data using the mobile based platform provided (MJaLi) Ensure the supported health facilities have the required equipment and supplies to facilitate quality hypertension management. This includes but not limited to functional blood pressure machines and data collection tools). Policies and protocols Support implementation of MoH polices at targeted health facilities with an emphasis on hypertension (and NCD) programs. Ensure health procedures follow national guidelines and protocols developed including those for quality assurance. Liaison with stakeholders and representation Collaborate with the County and Sub-County Health Management teams and other key stakeholders in project implementation. Representing the project/organization in stakeholders meetings and technical working groups as assigned. Continuous quality improvement Spearhead and participate in continuous quality improvement activities at the project supported sites to ensure provision of quality hypertension (and NCD) care. Integration of Project activities Work closely with staff at the supported sites to ensure hypertension service provision is integrated into other services. Participate in routine facility meetings so as to give feedback on performance of the program. Monitoring and Evaluation Participate in monitoring and evaluation of project activities to ensure that project objectives are met. Regularly review the work plans to ensure planned activities are implemented on time. Actively participate in data management at the facility and community level. Work closely with the Monitoring and Evaluation Officer and data clerks to ensure the quality data collected at health facility and community level is quality. Full Ensure data collected at health facility level is shared and discussed with the site staff and subcounty as appropriate. To work closely with health workers and CHVs to ensure that the register and patient forms are well filled Programmatic duties Participate in the development of the proposals, procurement, budgets and work plans and participate in organisational activities with guidance from the Project Manager. Reporting Ensure all the required reports are prepared and submitted on time. Support preparation of routine and adhoc Amref Health Africa in Kenya reports, donor reports and Ministry of Health reports. Research and documentation Participating in authorized research projects in the organization and best practices documentations and dissemination Any Other Tasks • Participate in various stakeholder forums and technical working groups as guided by the project manager. REQUIRED QUALIFICATIONS Education • A clinical officer or nurse with minimum undergraduate degree in health related field or social sciences. • Training in chronic disease management (diabetes, hypertension or HIV) • Training in monitoring and evaluation and is an added advantage Experience • At least 4 years working experience post-graduation • At least 2 years clinical practice experience working in a busy health facility managing chronic diseases including hypertension, diabetes or HIV • Experience with commodity management in a busy health care setting • Demonstrable experience carrying out capacity building of health workers • Demonstrable knowledge and experience implementing the Ministry of Health Community Health Strategy including interacting with Community Health Workers • Experience working in a multi- sectoral team is an added advantage Skills • Good interpersonal communication skills • Good analytical/problem solving skills. • Good organizational skills – planning and implementation • Innovation, creativity • Teamwork skills – A team player able to manage and coordinate teams • Report writing skills Competences • Ability to work well under minimum supervision • Highly adaptable, flexible and strategic planner • Able to network well with partners, stakeholders and colleagues at all levels • Able to identify skills gaps and opportunities for self-development to improve performance ENVIRONMENTAL CONDITIONS Corporate Policies, Systems, Procedures and Methods Thorough understanding of all Amref Health Africa Policies and Procedures - HR, Finance, Procurement, Transport, Per Diem, Communications, Asset and Disposal, ICT, etc. Should be a resident of Nakuru/Kiambu Work Environment Vast regions: Covering many health facilities across counties which may be far apart. Required utilize the means of transport available to reach the supported sites. The role is largely field based and no physical office space will be provided. The staff will utilize available space within the supported health facilities as required. Hours of Work 38 hours per week, Monday to Thursday with official working hours from 8.00am to 5.00pm with one hour lunch break and on Friday, the official working hours are 8.00am to 2.00pm
